Problem, Information, or Event Reporting
During the conduct of research, new information may become available or an event may occur that requires reporting to the IRB.
Principal Investigators are responsible for submitting a Report Form to the IRB according to the IRB's reporting criteria for certain types of problems, information, or events.
If the problem, information, or event meets UIUC's prompt reporting criteria, the Report Form must be submitted within 10 business days of the research team becoming aware of the problem, information, or event.
