Why do problems/events need to be reported?

Any events/problems that may be an unanticipated problem may involve risks to participants and others. The IRB needs information about these problems and events in order to determine that (a) risks to participants are minimized, and (b) the risks are reasonable in relation to the anticipated benefits.
