Where is the Research Equipment Form?


Answer

The Research Equipment Form is no longer available.  Generally speaking, the IRB only needs full details of any investigational devices being used.  Any other equipment or devices being used in the course of the study should be described within the New Study Application form as part of the procedures of the research.
