Summary

All student-led research projects (e.g. dissertations, theses, etc.) must be reviewed by the responsible department/college, in addition to the faculty advisor PI, prior to OPRS review of the project.  This review must be conducted by the applicable Department Head, Associate Dean for Research, or Dean.  This form should be used to document the departmental/college review and approval and the signed version should be attached to the IRB application in question #23.7 in IRBOnline.
