Summary

All human subjects research projects must be reviewed and approved before human subjects are recruited or involved in research activities. All projects must be presented on a New Study Application Form.

Helpful Tips

  • Do not use the same title for more than one project.
  • Avoid using technical jargon. Not all reviewers are conversant in your discipline.
  • A copy of the measures is required even in research involving open-ended, informal interview procedures.

IMPORTANT: The "New Study Application Form" replaces the previous "Protocol Form".  Please use this form to submit any new human subjects studies for review.

All human subjects research projects must be reviewed and approved before human subjects are recruited or involved in research activities. All projects must be presented on a New Study Application Form.

APPLICATIONS WILL NOT BE REVIEWED BEFORE THEY ARE COMPLETE.

The following materials must be submitted in order for the application to be complete:

  • The New Study Application Form, completed, signed by the PI, and submitted electronically.
  • The complete grant application, contract, deliverables, or other documentation of funding application for externally funded research.
  • Relevant consent forms and scripts.
  • All measures, such as surveys, interview protocols, inventories, and descriptions of experiments that are not already included on the Protocol Form under Procedures. Draft versions of these items are acceptable if the final versions are not available.
  • Recruiting Materials.

In addition, the New Study Application mentions additional forms that may also be required with the application, depending on the nature of the study and your responses to the New Study Application questions.  For example, the Vulnerable Populations form; the Devices form; the Databases and Repositories form; the Drug and Supplements form; the Informed Consent Process form; the Biological Materials form; and other forms might also be required, depending on your responses to certain parts of the New Study form.
